Yarnell Fire Investigative Report

On Saturday September 28 the Arizona State Forester released the Yarnell Hill Fire Serious Accident Investigation Report analyzing the circumstances leading to the June 30 entrapment and deaths of 19 firefighters of the Granite Mountain Interagency Hotshot Crew. Does any federal agency have authority to investigate this event?

Apparently lack of GPS and GIS info cost these firefighters their lives. IMO this was an OSHA violation subject to criminal penalties.

The lack of adequate Self Contained Breathing Apparatus at the 9/11 WTC location for Fire Service personnel and law enforcement was never the subject of an adequate investigation.
